Support Coordination Manager / Team Leader

Utilising your strong leadership skills and reporting to the CFO you will be responsible for the successful and timely delivery of support co-ordination, team development, co-ordinating and supporting current and potential National Disability Insurance Scheme (NDIS) participants within the NDIS to ensure they obtain the best possible support, and relationship building with external entities to both promote and drive new participants to the organisation's services.

Core duties include

  • Overseeing a small team, you will ensure the efficient, safe and effective delivery of a range of tasks within predetermined budget, quality, and time constraints
  • Engagement of Participants through working in partnership with participants and their family/support network to enhance their capacity to be more active in making decisions about their life
  • Supporting participants to strengthen their capacity to establish their network and maintain support services, links and relationships
  • Supporting participants to understand their NDIS plan, goals and objectives whilst promoting choice and control
  • Supporting participants to identify new goals and strategies to maintain social and economic participation and community access and inclusion
  • Assisting participants to identify early supports and preventative strategies and assisting where there are crisis situations or escalation of needs
  • Establishing a respectful and collaborative working relationship with participants and their family/support network that encourages and promotes choice, control and inclusion
  • Developing strong and collaborative working relationships with multiple services and service sectors
  • Assisting in the presentation of workshops for participants and their family/support network, to provide them with information to prepare for the NDIS
  • Ability to project manage the new aspects of the NDIS, along with the associated cultural change
  • Developing, enhancing and creating new relations with external bodies that promote the organisation and drive new participant numbers
  • To build strong relationships with National Disability Insurance Scheme (NDIS) representatives for assistance with guidance and support direction

Skills and core requirements

  • Degree qualifications preferred and advanced MS Office skills
  • Excellent interpersonal and communciation skills, both written and verbal
  • An understanding of the principles of the NDIS
  • Experience in the disability sector will be highly regarded
  • Demonstrated skill in navigating the multiple sectors and linking individuals to relevant services and supports
  • Marketing and promotion experience
  • Ability to network and develop effective working relationships and collaborations to achieve targeted interventions, referrals and support
  • Advanced computer skills in databases like a Client Information Management System (CMS)
  • Current full Victorian Driver's licence
  • Previous experience working with Committees
  • Ability to facilitate support coordination of participant plans and completion of all necessary documentation
  • Strong relationship building skills and an ability to engage with external organisations and enabling new participants to attend
