Our client is seeking a dedicated and compassionate Case Manager to join their team on a temporary 2-3 month basis starting in early June. In this autonomous role, you will play a crucial part in assessing needs and the development and delivery of home care plans to clients and their carers.

Key accountabilities:
  • Conduct initial assessments to determine client eligibility and primary needs
  • Undertake comprehensive assessments and create care plans
  • Provide information and advice to clients about self management and coordination of their care plans
  • Provide information and advice to clients about available service options
  • Develop individual budgets within the available funding to meet their needs and goals
  • Accurate updating of the client management system and meet payroll, finance and other deadlines
  • Undertake mandatory reviews as identified in the Care Plan
  • Effectively manage complex issues that involve competing demands or principles
About you:
  • Tertiary qualification in Health or a related field and evidence of relevant case management experience
  • Experience in assessments and developing care plans
  • Experience in setting and management of consumer budgets
  • Great communication skills with the ability to communicate effectively and compassionately with people from diverse backgrounds
  • Problem solving ability and a solutions focused approach 
  • An understanding of Aged Care Reforms and a passion for the industry
  • Familiarity with community aged care services and referral pathways
  • Exceptional customer service skills
  • Ability to work autonomously; sound time management skills and the ability to handle multiple priorities simultaneously
  • Preparedness to travel to regional Victoria and Northern NSW as required
Desirable
  • Understanding of the Commonwealth and Victorian Carer Recognition Acts
  • Understanding of the service and quality standards applicable to relevant programs, including State and Commonwealth frameworks (e.g. State Carers Program, Commonwealth Home Support Programme, NDIS etc.)
  • Understanding of Consumer Directed Care principles and applications
  • Experience working with people from diverse backgrounds and complex needs
