Key Responsibilities:
- Manage a caseload and deliver consumer-focused care plans
- Conduct assessments, reviews, and facilitate package upgrades
- Coordinate services and hospital discharges
- Build strong relationships with consumers, families, and stakeholders
- Ensure compliance with Aged Care Quality Standards
- Experience in case management or knowledge of Home Care Packages
- National Police Check (or willing to obtain)
- Driver’s licence + reliable, insured vehicle
- COVID-19 vaccination certificate
- Excellent communication, organisation, and MS Office skills
