Key Responsibilities:
  • Manage a caseload and deliver consumer-focused care plans
  • Conduct assessments, reviews, and facilitate package upgrades
  • Coordinate services and hospital discharges
  • Build strong relationships with consumers, families, and stakeholders
  • Ensure compliance with Aged Care Quality Standards
What You Need:
  • Experience in case management or knowledge of Home Care Packages
  • National Police Check (or willing to obtain)
  • Driver’s licence + reliable, insured vehicle
  • COVID-19 vaccination certificate
  • Excellent communication, organisation, and MS Office skills
